SickKids values your feedback. This online form is for patients, families, and visitors who want to communicate about their SickKids experience. You may submit compliments, concerns, suggestions, questions, or other related information.

In the case that your feedback contains personal health information, SickKids is committed to protecting your privacy in accordance with the Personal Health Information Protection Act (PHIPA).
