Breath Hydrogen collections are scheduled by faxing the requisition to Customer service: 416-813-5431 or 416-813-6599 

All requests require the following information:
Patient information: full name, DOB, home phone number, Health Card number, substrate required and weight.
Physician's name, address and fax number. 

Testing is only available for children over 2 years of age.
